Broad Street Pump is a good book about how the mystery of the Cholera outbreaks was solved by a determined doctor. During the 19th Century, there were 3 great pandemics that killed large numbers of people in Asia & Europe. The primary killer was cholera, a disease for which there was no known cure.
One physician who sought a cure for cholera was John Snow who was a disciplined individual who suffered from the disdain of the British medical profession. Snow's research led him to the conclusion that cholera's spread was through contaminated drinking water.
In Snow's "grand experiment" he identified a contaminated pump and disabled it and by doing so began the defeat of the disease in London. Snow's work also furthered the development of germ theory and helped debunk the widely held theory of spontaneous generation of diseases.
This is a good book.

This is a well-written, entertaining book. I read it mostly due to my interest in sea-faring and the "age of sail," as the author terms it. However, I found myself enjoying it just as much for the story it tells about the gradual discovery of a cure for a disease that crippled sea-faring nations for centuries. Particularly enlightening is the story of the bureacracy, the British Admiralty, that stubbornly ignored the potential cure, even as it suffered tremendous losses for its ignorance, and how vital privilege and influence is in challenging and changing such an establishment. The book's only minor flaw is that it focuses primarily on one country, Britain, without elaborating on how or why France or Spain failed to find a cause and a cure. I would recommend it highly.

I had a chance to read this book cover to cover. All I can say is "absolutely outstanding", short of calling it a historical masterpiece in the field. Very rarely do I encounter an epidemiology or biostatistic textbook that reads so well. It is optimally reader friendly; the author appears to have such a talent in explaining some most sophisticated epidemiological and statistical concepts in such a simplified language. Yet he does not sacrifice the inclusion of some very advanced epidemiological and statistical concepts. New concepts such as causal graphs and instrumental variables are also included and explained beautifully. I strongly recommend this book to all early to intermediate graduate students majoring in Epidemiology. Established epidemiologists may wish to read this book to refresh and update their knowledge. I hope the author writes more textbooks with the same style.

A statistician might find Gehlbach's explanations a bit too simplistic, but I think it gives a fantastic overview of how to approach interpretation of journal articles. An attending asked us to buy the second edition of this book when I was a 3rd Year med student, and I still use it today teaching residents. He does an excellent job using examples from the literature to illustrate his points. And, unlike other statistical texts, the index can help you grasp topics like the null hypothesis or type I error very quickly.

The Black Death struck Europe in the fourteenth century, killing hundreds of millions of people. There was little understanding of even the most basic causes and effects that might have been involved in the spread of the disease. It is not true, however, that the scientific insights about disease that were to start in the Enlightenment and continue to our own times represented the first understanding of the plague as something other than a magical curse or a lesson from God. In Italy, the plague recurred in an epidemic from 1575 to 1578, and during this particular outbreak, people began some primitive but essential epidemiology and even societal treatment. This is the surprising lesson of _Cultures of Plague: Medical Thinking at the End of the Renaissance_ (Oxford University Press) by Samuel K. Cohn, Jr., an astonishingly detailed look at the beginnings of an intellectual revolution. The author, a specialist in the Italian Renaissance and in the history of disease, reminds us that there was "no simple binary progression from a supposed premodern to modern medical thought." The doctors, medical workers, and even gatekeepers in the middle of the plague in Milan, Padua, and Venice observed, looked at the illness in new ways, and wrote about what they saw.
The writings are essential to Cohn's study, and represent a basic shift in the way the plague was documented. Barber-surgeons might write about the plague, and drug-sellers, merchants, lawyers, artisans, clerics, and minor officials all got into the act, a printed resource which Cohn has mined as the bibliographical backbone of his book, "a wide spectrum of authors from cardinals to cobblers, in prose and verse." The tracts from this time were not just in academic Latin but in the vernacular. Now doctors had a new confidence, and it was gained in part by disregarding ancient teachings. Experience and even experimentation began to be stressed. It seems like a small step, but doctors began documenting that mortality was occurring within households, getting an idea of the concept of contagion and disease transmitted from one person to another rather than being "miasmic," transmitted by air that was somehow bad. There were attempts to explain why the poor were disproportionately affected. One author wrote that plague struck the lower classes because they were "forced to eat bad and corrupt food, stinky bread, grass, and unknown roots". The poor could not count upon clean water; polluted water began to be recognized as a cause of disease. The poor lived in filth; there was lack of plumbing in their houses, buckets of excrement were kept under the beds, the windows (if any) were sealed, and dirt accumulated. Seeing the social effects of the plague in this new way resulted in new ways of treatment. "To judge from their published treatises," writes Cohn, "the crisis of 1575 fundamentally altered doctors' approach to and thinking about plagues from the individual patient to the community." The causes of plague were social, so the solutions had to be political. Recommendations were made about how the cities ought to issue decrees about administering the sick, building new hospitals, enacting quarantines, cleaning the streets, executing those who dealt in infected goods, and other public measures. At least some of these decrees were put into action, and in addition, charity to improve the lot of the poor was enforced. There was a place for religious processions and other ceremonies, but secular governments were in charge of making practical changes. The point of view developed in this outbreak attributed any triumph over disease to rulers and civil servants, the subjects of many subsequent literary celebrations.
Of course the doctors and citizens had no real knowledge of the disease roles of rats, fleas, and _Yersinia pestis_, but the capacity to look at clinical evidence without the burden of forcing it into ancient medical theories did enable some important changes, and was a foundation for better understanding as the centuries went on. Cohn, in an erudite and beautifully organized book, has cited a multitude of tracts, and quoted many of them here to show the beginnings of these initial ideas about how governments should involve themselves in the administration of health procedures for their citizens. Four centuries later, we are still trying to answer the same question for our own times.

Cultures of Plague opens a new chapter in the history of medicine. Neither the plague nor the ideas it stimulated were static, fixed in a timeless Galenic vacuum over five centuries, as historians and scientists commonly assume. As plague evolved in its pathology, modes of transmission, and the social characteristics of its victims, so too did medical thinking about plague develop. This study of plague imprints from academic medical treatises to plague poetry highlights the most feared and devastating epidemic of the sixteenth-century, one that threatened Italy top to toe from 1575 to 1578 and unleashed an avalanche of plague writing. From erudite definitions, remote causes, cures and recipes, physicians now directed their plague writings to the prince and discovered their most 'valiant remedies' in public health: strict segregation of the healthy and ill, cleaning streets and latrines, addressing the long-term causes of plague-poverty. Those outside the medical profession joined the chorus. In the heartland of Counter-Reformation Italy, physicians along with those outside the profession questioned the foundations of Galenic and Renaissance medicine, even the role of God. Assaults on medieval and Renaissance medicine did not need to await the Protestant-Paracelsian alliance of seventeenth-century in northern Europe. Instead, creative forces planted by the pandemic of 1575-8 sowed seeds of doubt and unveiled new concerns and ideas within that supposedly most conservative form of medical writing, the plague tract. Relying on health board statistics and dramatized with eyewitness descriptions of bizarre happenings, human misery, and suffering, these writers created the structure for plague classics of the eighteenth century, and by tracking the contagion's complex and crooked paths, they anticipated trends of nineteenth-century epidemiology.

This series is a long and successful collection of volumes on the most important topics in statistics. Professor Krishnaiah from the University of Pittsburgh was one of the first editors. When the famous Professor C. R. Rao moved from India to the US he joined his colleague Krishnaiah at Pittsburgh and helped coedit some of these handbooks. After Krishnaiah's death Professor Rao continued to edit these volumes and brought in some of his colleagues as co-editors.
This is volume 27 and it contains 27 chapters on the topic of medical statistics and epidemiology. This is very important to me as I work in the medical research area.
The editors have assembly the leading experts in statistical methodology and applied biostatistical reserach to produce these 27 chapters.
Ross Prentice (coauthor of 2 edition of a book on survival analysis and the Cox model) wrote an overview chapter on the topics to be covered. He has had a long career at the Fred Hutchinson Cancer Research Center in Seattle Washington. Fred Hutchinson was a famous manager for the Cincinnati Reds in the early 1960s and brought them to the World Series in 1961. He died of cancer and his estate started an endowment to form this research center.
Don Rubin covers causal analysis in Chapter 2. Rubin is an expert on causal inference, Bayesian methods and missing data. He presents the randomized trial approaches of Fisher and Neyman introduces propensity scores. A Bayesian approach is also covered.
Other chapters cover epidemiologic study designs, assessments for biomarkers, linear and nonlinear regression models, logistic regression, regression models for count data, mixed linear and nonlinear models, survival analysis, competing risk models, cluster analysis, factor analysis, structural equation models, crossover trials, group sequential methods, early phase trials (I and II), late phase (III and IV), missing data, Meta-Analysis, multiple comparisons, sample size determination
statistical learning using splines, evidence based medicine, marginal regression models, use of difference equations in modeling and the Bayesian approach to medical inference.
This volume is over 800 pages and each chapter is fairly long and includes large bibliographies.

For what this book intends, it truly is outstanding. In general, the medical researcher of any kind is at a severe disadvantage in dealing with professional biostatisticians because often the two do not speak a common language. This very readable, interesting and clearly written book gives the scientist enough knowledge to deal with most relatively simple cases in a language that we understand. With the basic principles of medical statistics made comprehensible, we are able to understand many of the simpler software packages well enough to perform basic statistical analyses and, more importantly, are able to converse intelligently with biostatistical professionals and to design studies that will pass muster. Unlike another reviewer, I do not feel that not covering SAS or other software programs is a disadvantage. No book can cover everything, and this book covers the basics and intermediate levels far better than any book I have seen, certainly better than any biostatistics book written by biostatisticians intended for other biostatisticians. This book understands that biostatistics is a tool, not a fundamental discipline, for the vast majority of scientists and is written to them. The book serves as a ready reference and review if one does not perform these analyses on a daily basis. This book should be on the shelf of every life scientist.

I ordered this book hoping for the kind of in-depth medical research and analysis that I enjoy reading. I was not disappointed. Each chapter is very thorough and detailed in its description and analysis of the "patient" and their condition. I can't state that the author has solved all these mysteries, but that's not what made this book enjoyable. It was the quality of medical information and the very scientific look at the lives of these historical figures. The details of their illnesses bring them to light as human beings rather than just figures of history.

This is a very well written and popular text on biostatistics. Altman writes for non-statisticians but the book is best suited for those with at least one prior course in statistics and those who have had mathematics through high school algebra. Emphasis is placed on the important practical problems. Good statistical designs and analyses are emphasized. The pitfalls with many published medical articles are discussed in Chapter 16.
I used this book to teach a 20 lecture course to students (engineers, clinicians and computer scientists) at Pacesetter in 1998 and at Biosense Webster in 1999 (both medical device companies that employed me as senior biostatistician). It was a good refresher course for the CRAs and engineers and it helped to make it easier for me to work with them on their statistical problems.
I have also taught a similar course to undergraduate students in the Health Science Department at Cal State Long Beach. Altman's book is a little too advanced to use as a text for that course but I did use it as a reference and covered material in Chapter 16 at the end of the course. Clear discussion of the medical literature is very important to these students and Altman does a great job!
